Data Subject Request Form

You may apply to the data controller, Furkan Yalçınkaya (Neffa Hair & Makeup Studio), to exercise your rights under Article 11 of Law No. 6698 (KVKK). Your application is concluded free of charge within 30 days at the latest; a fee from the tariff set by the Personal Data Protection Board may apply if the operation entails additional cost.

For your application to be assessed, it must include the following (Communiqué on the Procedures and Principles of Application to the Data Controller, Art. 5):

• Full name and, if the application is written, a wet signature
• Turkish ID number for citizens of the Republic of Türkiye; nationality, passport number or ID number for foreigners
• Residence or workplace address for notification
• If any, email address, phone and fax number for notification
• Subject of the request (clear and comprehensible)

You may submit your application through the following channels:

• By post: Send your wet-signed petition, together with a document verifying your identity, to Sapanbağları Mh., Yeni Mithatpaşa Cd. 15/A, 34400 Pendik / İstanbul.
• By email: Send it from the email address registered in our system to iletisim@neffahair.com.
• In person: Apply at our studio by presenting your ID.

An email application is valid only if sent from the email address you previously provided to us and which is registered in our system.

Your application is answered as soon as possible and within 30 days at the latest. If your application is rejected, you find the response insufficient, or no response is given in time, you reserve the right to lodge a complaint with the Personal Data Protection Board.
